THIS OSTEOPATHIC LIFE PHYSICIAN COACHING & CME PROGRAM PARTICIPATION AGREEMENT
Client-Coach Relationship. An effective coaching relationship requires mutual understanding and respect between the Coach and the Client. That requires the parties to agree to some basic tenets of coaching, including:
Coach and Client agree to communicate honestly, to be open to feedback, and to make time and space to participate fully in the coaching sessions in this program. Student specifically agrees to be open to coaching.
Coaching does not involve the diagnosis or treatment of any medical or mental disorders and does not prevent, cure, or treat any mental disorder or medical disease. Further, coaching is not a substitute for therapy, counseling, psychoanalysis, medical treatment, substance abuse treatment, or the advice or services of a medical professional.
The Coaches for the This Osteopathic Life Physician Coaching & CME Program are physicians, however, for the purpose of this program they are serving exclusively in the capacity of Coach. Participation in this program does not represent engagement in a physician-patient relationship. All information shared in coaching sessions will be held in confidence except where required by law. It is the Client’s responsibility to seek independent guidance from medical professionals to the extent necessary.
By participating in the coaching, I agree to accept personal responsibility for the results of my actions. I agree that the Coach has not made any guarantees about the results of taking any action, whether recommended during any coaching session or not. I recognize that my ultimate success will be the result of my own efforts, my particular situation, and innumerable other circumstances beyond the control and/or knowledge of the Coach.
All materials utilized in this course are for the personal use of the registered participant only and are not to be distributed externally or reproduced for any other purpose. I agree to comply with use of printed, video and audio materials provided by This Osteopathic Life within these parameters.
IN REGISTERING FOR THIS COURSE, I AGREE TO THE ABOVE AGREEMENT
